When you are initially chosen for the jury pool, you will have an opportunity to request to be excused. Say you have moved out of town, or you have a medical condition that would preclude you from participating, you will make that request with your initial questionnaire. You will need to provide proof in the form of an envelope or package label with your new address or a medical note. You will be notified whether you are excused for the year.
When you receive a summons for a specific trial date, you will have another opportunity to be excused for that specific date. You may request to be excused from that date should you have a vacation planned or a special event to attend or have a mandatory appointment or exam. You will fill out the affidavit on the bottom of the summons and return it to the Court with your excuse and any supporting documents (travel itinerary, etc.) Please note that a busy work or school schedule is typically an insufficient reason for an excusal.
All requests are reviewed by a judge and you must attend jury duty unless you receive confirmation from the court that you have been excused.
